We reserve the right to reschedule or decline appointment requests. Please contact us directly for urgent or emergency concerns rather than relying on an online form.

4. Cancellations and Missed Appointments

We ask that you provide reasonable advance notice if you need to cancel or reschedule an appointment. Repeated missed appointments or late cancellations may be subject to a fee, in accordance with our office policy, which will be communicated to you.

5. Fees, Payment, and Insurance

Fees for services are payable as arranged with our office. While we may assist with insurance claims and provide coverage estimates, any estimate is not a guarantee of coverage or reimbursement. Your benefit provider makes the final determination, and any portion not paid by your plan remains your responsibility. Coverage details, including CDCP and NIHB, are subject to the rules of the applicable program.
